When Should You Call Your Surgeon For Complications Following Endoscopic Sinus Surgery

• A sudden increase in the amount of bleeding from the nose unrelieved by pressure, ice, and head elevation.
• A fever greater than 101.5 F that persists despite increasing the amount of fluid intake and acetaminophen use.
• Persistent sharp pain or headache that is not relieved by the prescribed pain medication.
• Increased swelling or redness of the nose or eyes.
• Drainage of a thin, clear fluid in large quantities from usually only one side of the nose. This would be different from the clear, thicker mucus drainage normally produced by the nose.

What Happens During Sinus Surgery

What is sinusitis?

Sinusitis is a common condition in which the linings of the sinuses become infected and inflamed.

Your sinuses are small, air-filled cavities which are located at the front of the skull behind your cheekbones and forehead and are connected to the inside of your nose. The sinuses produce mucus which usually drains into your nose through small channels. If you have sinusitis, the lining of your sinuses becomes inflamed and swollen, causing symptoms including pain, a blocked nose, a reduced sense of smell or a discharge.

What causes sinusitis?

There are two main types of sinusitis:

The sinuses are air-filled spaces in the bones of your face and head. They open into your nose and produce a thick liquid called mucus to protect the inside of the nose. If your sinuses become blocked, swollen or infected, this is called sinusitis. You may have: pain, swelling and tenderness around your cheeks, eyes or forehead, a blocked or runny nose, a reduced sense of smell, mucus problems, a headache.

Sinusitis is usually caused by a virus. Other conditions such as allergies, nasal polyps and tooth infections can also contribute to sinus pain. If antibiotics, nose drops, sprays or tablets do not help, you may need an operation.

Surgery to treat chronic sinus problems is called functional endoscopic sinus surgery . You are given a general anaesthetic, which means that you are asleep and do not feel any pain. The minimally invasive surgery is all done inside your nose. You do not have any cuts or stitches on your face. The surgeon puts a tube called an endoscope with a light and camera at one end into your nostrils. They guide this tube to your sinuses and widen them by either: removing some of the blocked tissue, inflating a tiny balloon in the blocked sinuses and then removing it.

The operation is tailored to your individual needs. Your surgeon removes any polyps, small amounts of bone and swollen lining that are blocking the sinuses. Once your sinuses are unblocked, your symptoms should start to get better.

Initial Evaluation And Treatment

As with many disease processes, the history of a patient with sinusitis is probably the most important part of the preoperative assessment . All patients with severe or persistent symptoms should be evaluated, and many can be helped with advice and medical treatment.

The fiberoptic endoscope enables the surgeon to examine the nose in great detail and is an essential tool for diagnosis. Patients with seasonal or perennial rhinitis should be given advice about avoidance of allergens and treatment with topical nasal steroid sprays and antihistamines. Acute infective sinusitis is treated with antibiotics and vasoconstrictor nasal sprays. If medical treatment has failed, the patient may be a suitable candidate for an endoscopic procedure.

Who Makes A Good Candidate For Sinus Surgery

The ideal candidate is someone who suffers from chronic sinusitis, sinus infections, headaches, facial pressure, drainage, or teeth pain and has already tried a variety of remedies, including antibiotics, allergy medication, and sinus rinses.

Some patients have breathing issues due to a crooked septum, which makes half of the inside of your nose larger and can cause your turbinates to grow larger as well, blocking breathing.

You should be referred to an otolaryngologist to see if you need sinus surgery. For the best aesthetic results, find an otolaryngologist whos also a board-certified facial plastic surgeon.

They will likely request a CT scan to check for a deviated septum , enlarged turbinates , infected tissue, and polyps that might be blocking your nasal passages. Typically, surgeons will fix only what a patient complains about. For example, if the scan shows a deviated septum but the patient isnt having any breathing issues associated with it, they wont recommend a surgery that isnt necessary.

The exceptions are if were doing something cosmetic to the outside of the nose, we sometimes fix it on the inside tooand sometimes we have to fix the septum just to have access to the sinuses, says Dr. Undavia.

Candidates For Sinus Surgery

FESS is most successful in patients who have recurrent acute or chronic infective sinusitis. Patients in whom the predominant symptoms are facial pain and nasal blockage usually respond well. The sense of smell often improves after this type of surgery.

A CT scan before FESS is mandatory to identify the patient’s ethmoid anatomy and its relationship to the skull base and orbit. CT scanning also allows the extent of the disease to be defined, as well as any underlying anatomic abnormalities that may predispose a patient to sinusitis.

Patient selection therefore involves a thorough history and physical examination, a trial with medical treatment and, finally, CT scanning. The result is a highly selected group of patients who can expect an improvement of up to 90 percent in their symptoms.

In patients with nasal polyposis that is not controlled with topical corticosteroids, FESS permits the accurate removal of polyps using suction cutters.5 It is not known whether the disease-free interval is extended for patients having endoscopic ethmoidectomies for polyposis compared with conventional polyp surgery, but the postoperative discomfort is minimal.
