Alternatives For Gcs In Upper Airway Disease
In both rhinitis and rhinosinusitis patients, systemic GCS treatment is in general reserved for those in whom disease control cannot be obtained by baseline medical therapy . However, in AR, allergen immunotherapy is an alternative option for patients suffering from uncontrolled symptoms. AIT modifies the natural disease course and recent well-performed trials have demonstrated reductions in both symptoms and use of rescue medication in patients with AR for both the subcutaneous as well as sublingual administration route . One study from 1969 compared the efficacy of one depot MP injection with a pre-seasonal administration of an alum-precipitated pyridine extracted grass pollen immunotherapy and found similar results between the two groups in terms of symptom improvement . However, this paper already stated that the potential AEs of MP do not justify the use of systemic GCS for a condition such as AR. One large Danish registry study including almost 40,000 AR patients actually showed the oral steroid-sparing effect of subcutaneous AIT for seasonal AR with an annual mean of 1.0 steroid injections in patients receiving SCIT versus a mean of 1.6 injections in the non-SCIT group. Of the SCIT-treated individuals, 84% did not need GCS at all after SCIT treatment . Aasbjerg looked at the same registry to compare AEs and found that AR patient treated with systemic GCS showed more diabetes and osteoporosis than those treated with AIT as mentioned above .
Local Adverse Effects Of Steroid
We found one case report on gluteal subcutaneous atrophy that was seen after a depot steroid injection of triamcinolone for AR . A study of Laursen et al. investigated specifically the reporting of all AEs related to GCS injections for AR to the Danish Register for the Side-Effects of Drugs and evaluated the reported events consecutively for a 10-year period. The study demonstrated that one out of 11,785 injections came with any local AE. Most AEs were reversible and primarily skin related, such as skin atrophy.

First of all, I wanted to share with you what you are most likely to run into with Infections and Prednisone Side Effects. So, prednisone as we all know is an immunosuppressant. I took prednisone to decrease my immune systems attack on my platelets, my blood cells. And one of the things that it also did, was it changed the number of white blood cells. It shifted the way the white blood cells showed up when they would test my blood. I would have a really high total white blood cell count. But it wasnt because I had an infection. So, it looked like I had an infection all the time.
If you just went by that one number. If you werent a really highly trained doctor, you wouldnt know that you have to look at all of the other little counts of the white blood cells to know whether it was an infection or not.

Harm Of Gcs In Children
There is limited knowledge of risks of using systemic GCS in pediatric CRS or ARS compared to pediatric asthma. As an example, the Childhood Asthma Management Program trial followed the annual bone mineral accretion of 877 children with mild-to-moderate asthma . Oral GCS bursts produced a dosage-dependent reduction in bone mineral accretion and an increase in risk for osteopenia for 0, 14, and 5 courses, respectively, in boys. The authors conclude that multiple oral GCS bursts over a period of years can produce a dosage-dependent reduction in bone mineral accretion and increased risk for osteopenia in children with asthma. 780 children with asthma were followed for a mean of 4.3 years and it was shown that boys with lower vitamin D levels are significantly more susceptible to the negative effects of GCS on bone mineral accretion over time . Regarding studies investigating GCS AEs in upper airway disease, the trial from Ozturk also looked at self-reported AEs during the 15-day course of oral MP . In this trial no clinically significant AEs were reported. At the end of the treatment, the mean weight change did not differ statistically significantly between the groups. No data of monitored AEs, nor that of long-term outcomes, nor that of bacterial culture were available in this study.
How To Stop Prednisone:
We may have mentioned that when steroids are taken even for a short time, such as a week or ten days, the usual protocol is to start at a relatively high dose and then taper it down gradually. That reduces the risk of an unpleasant withdrawal reaction. Always ask the prescriber about the taper and follow the instructions.
A.M.S was not told how to stop steroids:
I was on 20 mg twice a day of prednisone for a sinus infection. Had I known anything about this horrible drug, I would have never taken the meds and let my sinus infection clear up on its own. That would have been better than these prednisone side effects.
I was not told to taper the dose, so I took as prescribed 20 mg twice daily for 7 days. The day after stopping, my whole body hurt to the touch, as if I was black and blue all over. I was swollen, red and had a lump on my neck, not to mention being very disoriented. I went back to the doctor and he insisted this had nothing to do with the drug.
I checked myself into the ER where they put an IV drip with Benadryl and the like. I was discharged that day. No change. Next day, didnt hurt to the touch anymore. New side effect rash from head to toe and severe indigestion. Following day, rash subsiding, indigestion getting better. Still feeling a bit loopy, but I am told by next week I should be back to myself again.
I am warning everyone I know not to ever take a steroid unless your life is in danger. It is a very scary feeling all for a sinus infection.

Inflammatory diseases of the nose and paranasal sinuses in children include upper respiratory tract infections, chronic rhinitis, ARS and CRS. ARS is defined as increase of sinonasal symptoms after 5 days of infection or persistent symptoms after 10 days and characterized by the sudden onset of two or more of the symptoms for less than 12 weeks . Bacterial infection is expected when at least 3 symptoms are present among which discoloured discharge, purulent secretion in nasal cavity, severe local pain with a unilateral predominance, fever, elevated C-reactive protein or erythrocyte sedimentation rate, and double sickening . The definition of pediatric CRS differs from adult CRS by the symptom of cough and is defined by the presence of two or more symptoms, one of which should be either nasal obstruction or nasal discharge with/without facial pain/pressure with/without cough, lasting for at least 12 weeks . The diagnosis is confirmed by either nasal endoscopy showing edema, purulent drainage or nasal polyps in the middle meatus or CT scan showing ostiomeatal complex or sinus opacification. Of note, the presence of nasal polyps is much less common in pediatric patients than in adult patients with CRS .

Antibiotics Steroids No Help For Sinus Infections
Just in time for runny nose season, new research suggests routine sinus infections arent really helped by antibiotics and other medicine thats often prescribed.
In the British study, people suffering from facial pain and a runny nose with greenish or yellowish mucous generally improved within about two weeks whether they took the standard antibiotic amoxicillin, steroid nose spray or fake medicine.
The results, based on patients reporting whether their symptoms had improved, echo previous findings in children.
Antibiotics, particularly the penicillin-like drug amoxicillin, are among the most commonly prescribed medicines for sinus infections.
Steroid sprays sometimes are used, but the study found they also were no better than dummy drugs, although they appeared to provide some relief for patients with only minor symptoms.
The study should lead to a reconsideration of antibiotic use for acute sinusitis. The current view that antibiotics are effective can now be challenged, particularly for the routine cases which physicians treat, said lead author Dr. Ian Williamson of the University of Southampton in England.
Physicians can focus on effective remedies that improve symptom control, which include ibuprofen and other over-the-counter painkillers, Williamson said.
Inhaling steam and squirting salt water into the nose to flush out thick mucous are among other methods that sometimes provide relief, he said.

- indigestion take prednisolone with food to reduce the chances of stomach problems. It may also help if you avoid rich or spicy food while youre taking this medicine. If symptoms carry on, ask your doctor if you may benefit from taking an additional medicine to protect your stomach.
- sleep problems take prednisolone in the morning so the levels are the lowest at bedtime
- restlessness if youre restless when youre trying to sleep, take prednisolone in the morning so the levels are the lowest at bedtime
- sweating a lot try wearing loose clothing, use a strong anti-perspirant and keep cool with a fan. If this doesnt help, talk to your doctor as you may be able to try a different medicine.
