  • When Antibiotics Are Needed

Historically, sinus infections, also called sinusitis, were often treated with antibiotics. But today, many allergists warn against the random use of antibiotics for a sinus infection.

Antibiotics can help eliminate bacterial sinus infections. But when a sinus infection is caused by allergies, a virus, or other causes such as a structural defect of the sinuses, an antibiotic will not help to alleviate symptoms.

The overuse of antibiotics is when they are prescribed for reasons other than when they are needed. Because of the common overprescribing of antibiotics for the type of sinus infections that do not warrant such treatment, many people have developed whats commonly referred to as antibiotic resistance.

When Do We Need Antibiotics For Sinus Infection

Antibiotics are not needed for many sinus infections, but your doctor can decide if you need an antibiotic. You doctor may recommend antibiotics if:

  • You have symptoms of a bacterial infection and you have not gotten better after 10 days, even with home treatment.
  • You have severe symptoms such as severe headache or facial pain, or you have other problems, such as pus forming in your sinus cavities.
  • You have had sinusitis for 12 weeks or longer .
  • You have a fever longer than 3-4 days.
  • Your symptoms get worse after initially improving.

Side effects can range from minor issues, like a rash, to very serious health problems, such as antibiotic-resistant infections and C. diff infection, which causes diarrhea that can lead to severe colon damage and death.

Nice Is Advising Healthcare Professionals To Tell Their Patients That A Sinus Infection Will Likely Clear

    27 October 2017

The final guidance, developed with Public Health England, makes recommendations for treating acute sinusitis.

In most cases, people who have sinusitis will start to feel better within two-to-three weeks. The infection is usually viral, which means antibiotics should not be routinely prescribed, the guidance says.

Instead, NICE says healthcare professionals should advise their patients on how to manage their aches and pains with paracetamol.

They should also tell them that there is no evidence oral decongestants or steam inhalation will make any difference. And inform them that they should seek further medical advice if their symptoms get worse, or last for more than three weeks.

Dr Tessa Lewis, GP and chair of the managing common infections guidance committee, said: We know that most people with sinus infections will recover in a couple of weeks without needing any antibiotics, but that doesnt mean we should be sending them home without any information or advice.

Health professionals can help their patients cope with this infection and the sometimes unpleasant symptoms it can cause. They should tell them that theyll probably be feeling this way for a while, and that unless they are very unwell, the best thing to do is to take paracetamol and take it easy.

Professor Gillian Leng, deputy chief executive at NICE said:Antibiotic resistance is one of the greatest dangers to our health, which is why we must all work together to fight it.

Different Types Of Antibiotics For Sinus Infections

The sinuses can get infected due to bacteria. As bacteria tend to thrive and multiply more in warm, moist and dark areas, sinuses are more prone to getting infected. Hence, bacterial sinusitis can be cured with antibiotics. Doctors usually prescribe a course of one to three weeks. An important note to make here is that the prolonged use of antibiotics can lead to bacterial resistance, thus use these medicines carefully.

What Tests Diagnose The Cause Of Sinus Infections And Sinusitis

Sinus infection is most often diagnosed based on the history and examination of a doctor. Because plain X-ray studies of the sinuses may be misleading and procedures such as CT and MRI scans, which are much more sensitive in their ability to diagnose a sinus infection, are so expensive and not available in most doctorsâ offices, most sinus infections are initially diagnosed and treated based on clinical findings on examination. These physical findings may include:

    • redness and swelling of the nasal passages,
    • purulent drainage from the nasal passages ,
    • tenderness to percussion over the cheeks or forehead region of the sinuses, and
    • swelling about the eyes and cheeks.

Occasionally, nasal secretions are examined for secreted cells that may help differentiate between infectious and allergic sinusitis. Infectious sinusitis may show specialized cells of infection while allergic sinusitis may show specialized white blood cells of allergy . Physicians prescribe antibiotics if the bacterial infection is suspected. Antibiotics are not effective against viral infections many physicians then treat the symptoms.

In addition, both rigid and flexible endoscopy has been used to obtain diagnostic material from sinuses. These procedures are usually done by an otolaryngologist under topical and local anesthesia. Occasionally, there may be a need to sedate the patient. Some investigators suggest that endoscopy specimens are comparable to those obtained by needle puncture.

Prednisone For Sinus Infection: Does It Work

Sinusitis and Its Treatment | Todd Kingdom, MD, Ear, Nose and Throat | UCHealth

A sinus infection, commonly known as sinusitis, is when your sinuses get infected, causing them to become swollen or inflamed. These are mainly due to bacterial, fungal, or viral infection, as in the common cold. Sinuses are the hollow parts or cavities within your skull, which are found behind the nose, forehead, and cheeks. These cavities are lined with a protective layer of mucus, meant to fight against external irritants like dust.

When your sinuses get inflamed, this can cause an excessive amount of mucus to build-up within your cavities. In turn, this causes pain and discomfort, making your head feel extremely stuffy and heavy. Sinusitis is quite a common condition in the US, with about 30 million people being diagnosed with it each year. Prednisone is a prescription steroid intended to reduce inflammation within the body. As sinusitis is an inflammation of the sinuses, does prednisone work in treating it? Lets find out.

How To Tell If You Have An Actual Sinus Infection

Even though we often say we have a sinus infection even if its just inflammation or an allergic response, there are symptoms of an actual infection that may be treatable with antibiotics. Nasal congestion and pain under the eyes or around the temples are, of course, main symptoms, but others include the loss of the sense of smell, green nasal discharge, mucus dripping down your throat, cough, fever, fatigue, sore throat, and even bad breath.

Sometimes, a sinus infection will clear up without intervention, but if you develop a fever of 100.4 degrees or higher, have your symptoms for 10 or more days, notice that your symptoms are getting worse and are not improved by OTC medications, or you have multiple infections in a years time, you should seek medical treatment as soon as possible.

We find it astonishing that some prescribers do not warn patients about the possibility of psychological side effects brought on by prednisone and friends. Even a short-course of high-dose steroid can precipitate symptoms. And not telling patients about gradual tapering borders on bad medicine. To protect yourself and your loved ones from such medical mistakes we suggest our latest book, Top Screw-ups Doctors Make and How to Avoid Them.

Psychological reactions such as insomnia and irritability are common, but they are certainly not the only prednisone side effects. Some of these adverse consequences, such as osteopenia, diabetes or glaucoma, appear only after long-term use of the drug. Others, such as confusion, high blood pressure or a drop in potassium, can occur on a shorter time frame. The most serious prednisone side effects that may happen within a week or two of use are dangerous blood clots , ulcers and infections.

In a double-blind placebo-controlled trial by Venekamp et al. 174 adult patients clinically diagnosed with ARS received either 30 mg/day prednisolone or placebo for 7 days. The incidence of gastrointestinal complaints did not differ between treatment groups.

In a large nested casecontrol analysis based on the UK General Practice Research Database, 2105 cases of upper gastro-intestinal complications were compared to 11,500 controls and then evaluated for exposure to certain drugs e.g. corticosteroid use. The adjusted OR for current use of oral GCS was 1.8 for upper gastrointestinal complications overall . No statistically significant difference could be objectified for lower versus higher dosage of GCS.
